In the quality management world, they talk about problems having a "common cause" or a "special cause". The media treats all these scandals like some one-off incident happened, and everyone is shocked and outraged...but they are slowly coming around to realize that these things are commonplace. When there is a "common cause" for a problem, the only way to get rid of it is to modify the system that created it, or you will see more of them occur.

I didn't know this was being done, but it seems to make all the sense in the world. One problem with hashtags is everyone using different ones to follow a story, an event, whatever.
