b) even if it was a big deal, there's no way to definitively say it was on purpose like seemingly everyone wants to do.

The defender couldn't see his target. If he had been looking at what he was going to hit, the odds of helmet to helmet would have been drastically reduced. But, the slow mo shows the defender ignoring technique and just leading with the crown of his helmet.
