What if two of the people didn't touch each other? What if couples were having sex in the same room and then swapped? What if two people had sex while a third participated, but not to that extent?

The same safe sex considerations that apply to one-on-one sex apply to group sex, for the same reasons. People who have group sex may have higher numbers of sexual partners, and may for this reason be more likely to have sexually transmitted infections (STIs) or to be at risk for STIs than those who have fewer sexual partners.
Group sex can occur amongst people of all sexual orientations and genders.

OK, so if we are going to accept wikipedia's definition (where I go to learn about the birds and the bees) then wearing sweatpants and taking Viagra with 50 of your closest friends definitely counts.Group sex is sexual behaviour involving more than two participants at the same time.
