When a pitcher is thrown out for intentionally hitting a batter, the manager is almost always thrown out too. One the one hand, Mikolas threw at Happ twice. On the other, there was no warning issued after the first. Furthermore, once he was hit, as Chip pointed out, the Cubs didn't say a thing. Baseball justice was meted out. Happ takes his base, warnings are issued, and the game continues.
IMO, Barksdale and crew escalated the situation with the no-warning ejection. And then get Oli while he was calm. He must have said a magic word. The umpires inserted themselves into the game unnecessarily. Game management 101 and this crew failed IMO.
Further, I wonder if Oli is a marked manager. I wonder if the way he handled the CB Bucknor situation has the umpires looking for him. The Cardinals seem to consistently get bad calls against them. Yeah I'm a fan so I'd like to see if statistics bear that out. But its one more reason to be rid of Oli if true.
He and Bucknor did shake hands before a game recently.

You'd think so but with no runner on, there is no penalty. If runners had been on it would only be a dead ball and runners return.
